- Elimination Periods: 60 days, 90 days, 180 days
- Benefit Durations: 2 years, 5 years, to age 65 compliant with Age Discrimination in Employment Act (ADEA) and Social Security Normal Retirement Age (SSNRA)
- Survivor Benefits: 3 months, 1 year, 2 years
- Mental Nervous/Drug and Alcohol: 24 months
